Health Forms
A health assessment dated within 12 months is required for anyone attending Landmark Summer Programs. If you are a current student at Landmark Preschool, and your health assessment is dated within 12 months, you do not need to submit a new one. All health assessment forms are due to the office no later than Friday, June 5. If needed, Health Assessment forms for preschool students can be found here.
